The Fertility Coalition is committed to protecting the privacy of personal information and upholds the privacy requirements of the Privacy Amendment (Enhancing Privacy Protection) Act 2012 and other relevant legislation. This Privacy Statement (‘the Statement’) applies to the Your Fertility website and outlines the personal information collected through this website, why the information is collected and how it will be used. The Your Fertility website is managed by the Victorian Assisted Reproductive Treatment Authority (VARTA) on behalf of the Fertility Coalition.

Personal information

Personal information may be collected via the website including, name and contact details relating to enquiries or feedback via Your Fertility’s online enquiry form. The Fertility Coalition also collects data through Google Analytics and/or Piwik Analytics designed to track the number of unique visitors, time and date of visits to the site and behaviour of visitors on the website, such as page visited, clicks and referral sources. The information collected by Google Analytics and/or Piwik Analytics does not identify a user. This information is used for statistical purposes and to ensure our web site is providing information relevant to our users. The Fertility Coalition may distribute aggregated statistical information for statutory reporting purposes, but only in a form that will not identify any person individually.

How and why personal information is collected on this website

Personal information collected through the online enquiries form is used to provide advice, respond to positive and negative feedback and enhance Your Fertility’s services. Personal information collected through the online Preconception checklist is recorded in a secured database independent of the website and used solely for the purpose of distributing the results and requested information as specified by the user. We will only use personal information provided for the purposes for which it was collected.

Use and disclosure of personal information

The Fertility Coalition will not make available to a third party any personal information supplied by you unless required by law. There are no other additional uses other than the uses outlined above.

Access

The Fertility Coalition applies privacy and security measures to protect against misuse, loss, unauthorised access or unauthorised disclosure of personal information including:

  • Only authorised staff members are entitled to access your personal information and for approved purposes only.  In addition, all staff signed a confidentiality agreement as a condition of their employment.
  • Hardcopies of personal and sensitive information being stored in locked filing cabinets.
  • Passwords and security on all computers to protect electronic copies of personal information, including emails.

Security

Once any personal information is received by the Fertility Coalition, all reasonable steps are taken to protect that information from misuse, loss, unauthorised access, modification or disclosure. While the Fertility Coalition strives to protect personal information, the Fertility Coalition cannot ensure or warrant the security of any information transmitted online and individuals do so at their own risk. If you have any concerns about conveying sensitive information over the Internet, you can contact the Fertility Coalition.

Cookies

In addition to data collected by Google Analytics and/or Piwik Analytics, The Your Fertility website also uses cookies for some parts of this website. Most browsers will allow some or all cookies by default. However, you can take steps to erase cookies, block cookies or receive a warning before a cookie is stored, by altering your own browser’s cookies settings (usually through the Tools options). The Fertility Coalition recommends that users read the Privacy Statements of any external websites that the Your Fertility website may provide a link to. You acknowledge and accept that your use of the Your Fertility website indicates your acceptance of the terms of use outlined in this Privacy Statement. This Privacy Statement replaces any other Privacy Statement previously published on the Your Fertility website.

We may at any time vary the Privacy Statement by publishing the varied Privacy Statement on our website. You accept that by your use of the Your Fertility website after any variation, we have provided you with sufficient notice of the variation and you are taken to have accepted the revised Privacy Statement.
